בָּרוּךְ אַתָּה ה׳ אֱלֹקֵינוּ מֶלֶךְ הָעוֹלָם, בּוֹרֵא פְּרִי הָעֵץ

Baruch atah Hashem, Elokeinu melech ha'olam, borei pri ha'etz.
Blessed are You, Hashem our God, King of the universe, who creates the fruit of the tree.

Fruit that grows on a tree whose trunk stays from year to year gets Borei Pri Ha'etz.

Grapes are one of the seven species, so the bracha after them is Al Ha'etz.

After eating: The short three-in-one blessing, Al Ha'etz, after eating a kezayit of fruit from the seven species.

This follows the common ruling of most poskim. For different customs or a specific question, ask a rabbi.
